Elder Care -- Geriatric Care Manager

The Aging Life Care Association defines a Geriatric Care Manager as a health and human services specialist who helps families who are caring for older relatives.  The Geriatric Care Manager is trained and experienced in any of several fields related to care management including but not limited to nursing gerontology, social work, or psychology, with a specialized focus on issues related to aging and elder care.

  • Conduct initial care plan assessment to identify current status and problems or areas of concern
  • Develop personalized care plan based on initial assessment, and assist with putting the care plan in place
  • Provide consumer education, resources, and advocacy
  • Act as a “surrogate daughter” to help families at a distance with the coordination of care of their loved ones, and keep them informed of care needs, problems, or change in status.
  • Assist with finding healthcare providers and other resources specializing in geriatric care

Case Management

A case manager can be an invaluable member of the treatment team and assists with expediting services and coordinating care. Case management services are most effective when the case manager works as the liaison person between the client, attorneys, physicians, payer source, employer and other service providers.
The goal of the case manager is to assist the client in receiving quality care, and help them to achieve his or her maximum level of functioning.

The Commission for Case Management Certification (CCMC) defines case management as a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client’s health and human service needs. It is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes.

Med-Legal Healthcare Consultants provide case management services based on the level of assistance needed or requested. This can be as simple as attending a one time medical appointment with the client to receive an updated status, or being called in to assist with coordination on a catastrophic case.

Life-Care Planning

A Life Care Plan projects the anticipated costs and future medical care and services for a catastrophically injured or ill patient. This is used to help settle lawsuits by providing information to the Attorneys, insurance company and family, that accurately project the financial cost of rehabilitation and future care following an accident, or injury.
